What Is Torticollis? Head Tilt in Infants

Torticollis and head turning to one side in infants: investigating the causes, assessing movement, and follow-up with family involvement.

Updated: · General information

What is it?

Torticollis may be noticed as the head being held tilted or turned to one side. Alongside muscular causes, visual and neurological causes are also considered.

    How is it assessed?

Neck movement, head shape and motor development are examined. If there is newly developed painful neck deviation, fever or trauma, medical assessment is required first.

      How is rehabilitation planned?

Age-appropriate positioning, encouraging the child to look both ways during play, and family training may be used. Stretching technique is demonstrated by the physiotherapist; forceful neck movements are not performed.

Is head tilt always due to muscle shortening?

Torticollis describes the head being held tilted or turned. Although it can be muscular in origin, visual, neurological and bony causes must also be considered. Parents may notice that the baby always turns the head to the same side, prefers one side during feeding, or has an asymmetrical head shape.

How can the family take part at home?

The age-appropriate play, handling and changes of position demonstrated by the specialist are built into the daily routine. The neck is never turned by force. Supervised activities while the baby is awake must be kept distinct from sleeping position; safe-sleep advice remains in place. Range of movement and developmental milestones are reassessed at review appointments.

            Your questions

            Patient information

Should we wait for the child to grow before starting treatment?

Where there is a persistent side preference or head tilt, early assessment is appropriate. It is not right to rely on massage alone and wait for it to resolve without establishing the cause.

Is a suddenly developing neck tilt the same condition?

A new head tilt, particularly one beginning with trauma, fever or marked pain, may have different causes and requires medical assessment.
